
Raphael Project: Developing the integration of heart failure and palliative care services
INTRODUCTION
The RAPHAEL project aims to integrate a palliative care approach for heart failure patients across Europe. Through remote monitoring of symptoms at home, it seeks to improve patients’ quality of life (QoL) and quality of death (QoD) by offering a multidimensional and personalized treatment.
GOALS
- Improve the QoL and QoD of patients and caregivers.
- Reduce hospitalizations and healthcare costs.
- To provide access to palliative care to all patients, including rural areas.
METHODOLOGY
The project uses a telemonitoring application, PAL@HEART, to track patients’ physical and psychological symptoms at home. Patients, their caregivers and healthcare professionals collaborate on a personalized multidimensional cure plan.
RESULTS
This intervention is expected to reduce hospitalization and improve patients’ autonomy, while providing emotional, social and existential support. In addition, the results will help create a scalable model for application across Europe.
